POSITION: REGISTERED PRACTICAL NURSE

UNIT: CONTINUING CARE CENTRE (CCC)

STATUS: TEMPORARY FULL TIME (Approximately September 19, 2022 to November 1, 2023; may end sooner/be extended)

SALARY: $31.23 - $31.57

 

POSITION DESCRIPTION: 

The Continuing Care Centre (CCC) is committed to compassion and excellence in the provision of complex care and rehabilitation services. We strive to meet the physical, emotional, social and spiritual needs of each person. We believe in treating each person with respect, dignity and compassion.

Three levels of care are provided in our CCC unit that include:
  • Activation / Restoration – with a functional goal of progression (i.e. exercise and recreational activities) to increase and strengthen independence.  The average length of stay in this program is 56 to 72 days.
  • Short Term Complex Medical Management – with a functional goal of stabilization and progressions (i.e. medically complex and specialized services) to help avoid further loss of function, increase activity tolerance, and progress. The average length of stay in this program is up to 90 days.
  • Long-term Complex Medical Management – with a functional goal of maintenance (i.e. medically complex and specialized services over an extended period of time) to maintain/slow the rate of, or avoid, further loss of function.

The RPN works at their full scope of practice to provide direct care to assigned patients. The RPN practices independently when the patient’s condition is less complex such that the outcome is predictable. The RPN collaborates with the Registered Nurse in patient situations that are beyond the RPN scope of practice.


HOURS:

The successful candidate must be available to work all tours applicable to the department. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department. 

SKILLS & QUALIFICATIONS REQUIRED:

  • Registered Practical Nurse, currently registered and in good standing with the College of Nurses of Ontario practicing at full scope.
  • Preference will be given to those with at least one (1) year Continuing Care experience within past three (3) years
  • Current BCLS certification
  • Palliative Care Fundamentals (Level 1) preferred
  • Current Safe Management Training Certification preferred
  • Continuing education in geriatric nursing preferred
  • Recent experience in physical assessments, IV medication administration, administration of blood and blood products, catheterization, chest and respiratory assessment with the ability to demonstrate competencies;
  • Thorough understanding and commitment to Patient and Family Centered Care principles and ability to use in practice.
  • Demonstrated problem solving, critical thinking, decision making, and conflict resolution skills and able to work through conflicts and differences in a respectful manner.
  • Demonstrated excellence in the highest level of communication skills and customer service skills.
  • Demonstrated knowledge and commitment to patient and staff safety.
  • Ability to adapt and work within a constantly changing environment.
  • Basic computer skills (Outlook, Cerner, etc.)
  • Ability to meet the physical demands of essential duties of the position
  • Demonstrated ability to attend work on a regular basis

Successful candidates must provide evidence of a fully completed COVID-19 vaccination status prior to your first day of work.
