POSITION DESCRIPTION:
As a member of an inter-professional healthcare team, the Pharmacy Intern will provide pharmaceutical care, under the supervision of a Pharmacist. Duties will include but will not be limited to:
- Monitoring of clinical pharmacotherapy, drug information questions, and adverse drug reaction reporting.
- Collaborate with physicians, residents and interdisciplinary team members.
- Will require order review and verification of the physician computerized medication orders within the CERNER electronic patient record, document interventions, pharmacy progress notes, and update pharmacy organizer as required.
- Must be available for consult and be actively engaged on inpatient units.
- Required to participate in quality assurance and medication safety projects as required
- Provide discharge medication reconciliation and education to patients and connect with community pharmacy partners as required
- Assist in obtaining best possible medication histories and admission medication reconciliation as required
HOURS:
The successful candidate must be available to work during the week, on weekends and statutory holidays. The intern will only work shifts that can be under the supervision of a Pharmacist (shifts excluded are: 0700-1500 on weekends and holidays and 1100-1900 shifts during the week).
SKILLS & QUALIFICATIONS REQUIRED:
- Completion of University of Toronto or University of Waterloo Undergraduate Pharmacy Program
- Previous hospital pharmacy experience
- CERNER electronic health record experience preferred
- Demonstrated ability to work independently and as a member of a team
- Proven ability to prioritize workload
- Good written and verbal communication skills required, in order to interact effectively with patients, peers, and other health care team members
- Demonstrated communication skills in providing pharmaceutical service to patients; including clear and concise verbal and written communications
- Demonstrated ability to identify, prevent and resolve actual and/or potential drug-related problems
- Effective interpersonal skills
- Excellent attendance and discipline-free record required
