#040-20 RN, Community Withdrawal Management Program, Temporary - approximately 6 months, Choices for Change

REGISTERED NURSE – Community Withdrawal Management Program  - approximately 6 months

POSITION SUMMARY:

The incumbent will provide assessment, treatment and follow up for people living with a mental illness and/or addiction, with a focus on withdrawal management. The position involves working within a case management framework collaboratively with Nurse Practitioners, community mental health and/or addiction teams, primary care physicians, psychiatrists, family health teams and other community health service providers to support the health needs of individual patients living with mental illness and/or addiction, including those accessing methadone and suboxone at local clinics.

QUALIFICATIONS:
• Registered Nurse (BScN Degree) in good standing with the College of Nurses of Ontario
• Has purchased liability protection under RNAO or independently
• 2-5 years’ experience working in the Mental Health/Addictions field an asset
• Knowledge of working with individuals experiencing withdrawal from substances an asset
• Knowledgeable about primary health needs in persons living with a Mental Illness and/or Addiction
• Excellent health assessment skills, sound clinical judgment and decision making skills
• Ability to provide education to staff, clients and community members about health and wellness
• Demonstrated ability to take initiative and develop new programs to meet identified gaps in service.
• Knowledge of Information Technology systems (e.g. Ontario Telemedicine Network equipment knowledge)
• Ability to work collaboratively with patients, families, health care providers/teams and other members of the community
• Excellent communication, organization and interpersonal skill.
• Fluent computer skills/knowledge of Outlook, Excel, PowerPoint, electronic client records.
• Satisfactory criminal reference check and vulnerable sector screening
• Able to work flexible shifts
• Have valid driver’s licence and access to a vehicle with auto insurance

POSITION RESPONSIBILITIES:
• Responsible for the delivery of high quality nursing care
• Advocate on behalf of clients with other service providers, maintaining collaborative working relationships
• Maintain administrative and clinical records, including client files and data entry to Catalyst, as per Policy and Procedure Manual
• Protect the confidentiality of clients
• Engage in community awareness, education and outreach as designated and appropriate, representing the agency in a professional manner.
• Participate in and support the use of OTN technology within the agency and community
• Assist with program development, implementation and evaluation
• Support the ongoing development and evaluation of a Community Withdrawal Management Program for Huron Perth
• Participate on internal agency committees and/or outside community committees as appropriate
• Adhere to policies and protocols as outlined in the Policy and Procedure Manual.
• Attend and actively participate in any team and staff meetings, the Annual General Meeting and periodically scheduled strategic planning and team building events
• Assist with the orientation and training of new agency staff, volunteers and interns/students as designated and needed
• Participate in training to further develop skills required to perform the duties of the position
• Participate in ongoing evaluation of service delivery and personal performance
• Actively participate in the supervisory process with immediate supervisor
• Support and actively participate in the constant evolution and growth of the agency
• Perform other duties as assigned or required
